A fairly complex questionnaire/survey creation system using a client/server model. The system incorporates quite sophisticated scripting and customisation so that the survey can adapt to previous answers, perform loops, etc.

Written using my currently preferred development platform: Apache, PHP, MySQL, JavaScript, AJAX, HTML, CSS.
